Kindly give 24 hours notice if you are unable to make an appointment. A $25 cancellation fee may apply if no notice is given.
Co-pays are due at the time of visit.
Please remember to bring your referral if your health insurance company requires it. Wear loose fitting comfortable clothes for all of your appointments.
The initial evaluation will take 30-45 minutes. Subsequent therapy sessions typically take between 60 and 90 minutes.
